Important Notice
The following Equipment Release Form must be signed by the child’s legal guardian for C.A.T.S. (Children’s Assistive Technology Service) to process your equipment request.
Equipment Requests
Equipment must be requested by the child’s legal guardian. A referring healthcare provider (occupational therapist, physical therapist, or speech therapist licensed in Virginia) must be identified as part of the request. The referring provider may be contacted to verify the appropriateness of the equipment for the child.
How Equipment is Issued
Adaptive equipment (such as bath chairs, wedges, and positioning devices) and rehabilitation equipment (such as wheelchairs, standers, and gait trainers) are issued through C.A.T.S. based on availability and clinical appropriateness.
Once equipment is assigned, the legal guardian will be notified and must respond within five (5) business days to arrange pickup or delivery. If no response is received within this timeframe, the equipment may be returned to inventory and reassigned.
Unscheduled pickups are not permitted. All pickups and deliveries must be coordinated with C.A.T.S. staff.
Limitations of Services
- A child may receive up to 3 pieces of equipment through C.A.T.S. in a calendar year.
- C.A.T.S. does not provide warranty or repair services for any equipment issued.
- Equipment is provided to serve the child’s immediate needs and is not intended to address future or anticipated needs.
- C.A.T.S. reserves the right to deny or limit equipment requests at its sole discretion.
Use of Equipment
Upon receipt of equipment, ownership transfers to the legal guardian. The legal guardian assumes full responsibility for the proper use, maintenance, and supervision of the equipment.
Equipment received through C.A.T.S. may not be resold, transferred to another party for compensation, or used for any commercial purpose.
Waiver of Liability
By signing this Equipment Release Form, I, the undersigned legal guardian, acknowledge and agree to the following:
- I understand that C.A.T.S. provides equipment on an as-is basis, without any express or implied warranties regarding the condition, fitness, or suitability of the equipment for any particular purpose.
- I assume all risk associated with the use of the equipment, including but not limited to any risk of injury, damage, or loss arising from the use or misuse of the equipment.
- I release and hold harmless C.A.T.S., its officers, directors, employees, volunteers, and agents from any and all claims, demands, actions, causes of action, damages, costs, and expenses (including attorney fees) arising out of or in connection with the equipment, including but not limited to claims related to equipment condition, FDA recalls, repairs, modifications, or personal injury.
- I understand that it is my responsibility to ensure the equipment is used safely and in accordance with any manufacturer instructions or guidelines.
- I agree to notify C.A.T.S. promptly if any safety concerns or defects are identified with the equipment.
This waiver and release is binding upon me, my heirs, executors, administrators, and assigns.
